If you live in Seven Hills, Anthem, Sun City Anthem, or Anthem Highlands, use the Henderson office address when arranging an in-person visit. The practice does not have a separate neighborhood office. Before your first visit, ask which records to bring and whether any testing needs to be arranged separately.

If you are transferring care, ask how to share prior testing and current prescriptions through the practice’s secure process. Do not send medical records through ordinary email.

You must be physically located in Nevada for the practice’s Nevada telehealth service. Some concerns still need an exam or in-person care. Ask what can be handled remotely before assuming a video visit can replace an office visit.
Lake Las Vegas, Cadence, and Tuscany residents can use the same Henderson office and ask about eligible video follow-up.
